Back acne, commonly known as “bacne,” affects millions of people and can be just as frustrating as facial breakouts. While it may seem stubborn and challenging to treat, board-certified dermatologists Dr. Joel Schlessinger and Dr. Daniel Schlessinger at Schlessinger MD have developed proven strategies to help achieve clearer, healthier skin on your back. With the right approach and consistency, back and shoulder acne is absolutely treatable.

Why back acne develops
Understanding different types of back acne
Recommended treatment approach
Addressing back acne scars
Final thoughts

The skin on your back is uniquely problematic when it comes to acne. It’s thicker than facial skin and contains numerous large sebaceous glands that produce sebum, a type of oil.

“Several factors contribute to back acne development,” explains Dr. Joel Schlessinger. “Hormonal fluctuations, including testosterone or creatine supplementation, affect sebaceous gland activity, while genetics determine your skin’s predisposition to acne. Environmental factors like excessive sweating and friction from clothing or sports equipment create the perfect conditions for breakouts to develop. Lastly, infections can arise when working out or touching mats or other equipment during yoga or floor-based exercises.”

Understanding different types of back acne

Not all back acne is the same, and this distinction is crucial for determining the proper treatment approach. Dr. Daniel Schlessinger notes: “In practice, I see patients with various forms of back acne, each requiring a tailored approach. Comedonal acne presents as those familiar blackheads and whiteheads, while inflammatory acne includes the red, tender papules and pustules that patients find most bothersome. The more severe cases develop nodular or cystic acne, which penetrates much deeper into the skin and often leads to permanent scarring if left untreated.”

The cases that concern dermatologists most are nodular and cystic acne lesions. These penetrate deep into the skin and almost always lead to permanent scarring if left untreated. Early intervention is crucial for these deeper lesions.

Sometimes, what patients think is acne is actually something entirely different. Dr. Joel Schlessinger notes that he has diagnosed everything from bacterial infections to hidradenitis suppurativa in patients who came in thinking they had severe back acne. This is why professional evaluation is so important when over-the-counter treatments aren’t working.

Recommended treatment approach

Starting with over-the-counter solutions

For mild to moderate back acne, dermatologists recommend starting with proven over-the-counter ingredients. Salicylic acid is excellent for unclogging pores and reducing inflammation, making it a top recommendation for comedonal acne.

“I always tell patients to start with a body wash from CLn, which contains Sodium Hypochlorite, a diluted component in bleach,” advises Dr. Daniel Schlessinger. “The key is proper application technique. Apply the cleanser to affected areas and let it sit for 3-5 minutes to allow the active ingredients to penetrate. Then, rinse thoroughly. Consistency is crucial for achieving results.”

Glycolic acid and lactic acid can also help exfoliate dead skin cells and prevent future breakouts. However, dermatologists caution patients about over-exfoliation, which can actually worsen acne by causing irritation.

When to consider professional treatment

Clinical experience has shown that timing is everything with back acne treatment. Dr. Daniel Schlessinger has a clear guideline for his patients: “My general rule is this: If you’ve been consistent with an over-the-counter routine for several weeks without seeing any improvement, it’s time to come in. What looks like acne could actually be a bacterial infection, such as Staph or pseudomonas folliculitis, which requires different treatment. It could also be yeast (pityrosporum) folliculitis, which also requires an oral treatment.”

Back Acne Treated at Schlessinger MD

Even for moderate to severe cases of acne, dermatologists often prescribe topical retinoids, which work by accelerating cellular turnover and preventing the formation of microcomedones, which are the precursors to all acne lesions. Oral medications like antibiotics or isotretinoin (Accutane) may be necessary for stubborn cases.

Addressing back acne scars

“When it comes to scaring, the importance of early intervention can’t be dismissed,” says Dr. Joel Schlessinger. “Once scars have formed, we can address them with certain treatment options, but completely getting rid of bacne scars completely is very difficult, if not impossible. Prevention is always preferable, which is why an early visit to your dermatologist is so important. Dermatologists have access to tools such as Accutane, a powerful derivative of vitamin A, that has been clinically proven to reduce or even put acne into remission. I frequently use it on my most challenging back acne patients. Lastly, I often see patients who have been misdiagnosed as having back acne, but instead have a condition called hidradenitis suppurativa, which is treatable with a variety of newer medications that are not commonly used for acne.”

For existing scars, several professional treatments are available. Professional treatments also include chemical peels and Diamond Glow microdermabrasion. These treatments utilize higher concentrations of active ingredients than what’s available over-the-counter and can penetrate deeper into the skin.

Due to the extended area of the back, these treatments can be expensive, so dermatologists always discuss realistic expectations and treatment timelines with patients.

The path forward

Successfully treating back acne requires patience, consistency and often a combination of approaches. Whether you’re dealing with occasional breakouts or persistent cystic acne, effective treatments exist for every severity level.

Most treatments require at least six to twelve weeks to show significant improvement. Dermatologists always remind patients that skin care is a marathon, not a sprint. Stay committed to your treatment plan, make necessary lifestyle adjustments, and celebrate small improvements along the way.

If your back acne is affecting your confidence or quality of life, don’t hesitate to seek professional help.

Signs of acne don’t always go away when a breakout clears. Sometimes, you may be left with lingering discoloration or scarring, which can be even more difficult to treat than the breakouts themselves.

Board-certified dermatologists, Mohs surgeons and cosmetic surgeons Drs. Joel and Daniel Schlessinger are well-versed in improving acne and any lingering signs. In this blog, we’ll discuss the ins and outs of acne scarring, including:

Everything you need to know about acne scars

Acne scarring is typically caused by deep, cystic acne. However, untreated breakouts may also cause acne scarring. That’s where the most important kernel about acne scarring comes from—an ounce of prevention is worth a pound of cure.

If you’re experiencing new or worsening breakouts, it’s important you see a dermatologist as soon as possible. “There is no benefit to leaving acne alone or letting it run its course. Acne treatments have gotten better over the years, and there is no reason nowadays to let acne progress to the point of scarring,” Dr. Daniel Schlessinger says.

However, if you have an active acne breakout and acne scarring, the active acne must be treated before the scarring can be addressed. “I like to compare this to trying to clean up from a fire while the fire is still burning; you need to fully put out the fire first before picking up the pieces it left behind. The same goes for acne scarring,” Dr. Daniel Schlessinger says.

There are different types of acne scarring, though their types (icepick, boxcar or rolling) are mostly relevant to dermatologists, as some respond better to certain treatments than others. Acne scars can either be raised (hypertrophic or keloidal) or atrophic (indented or depressed). What treatments you and your dermatologist decide upon may depend on the type of scarring you have.

Microneedling for acne scars

Microneedling is a common procedure for people with textural concerns and is a minimally invasive treatment that superficially punctures the skin with small needles to stimulate the body’s wound-healing response. This treatment can help improve collagen and elastin production in the area.

While microneedling on its own may provide slight improvements, it shows the best results when combined with other treatments. “We do microneedling treatments with the ENDYMED System, which uses radiofrequency to stimulate collagen. Many of the acne scarring cases we see are a result of collagen loss, which is why ENDYMED can help,” Dr. Joel Schlessinger says.

He continues, “AQUAGOLD® Fine Touch Microneedling is another option, which combines microneedling with fillers, like Juvederm or Restylane, but it may also contain BOTOX®, which can help remodel scars.”

We recommend visiting a licensed medical aesthetician or board-certified dermatologist for a microneedling treatment in any variety to prevent additional scarring or skin damage.

Chemical peel for acne scars

Chemical peels are another common treatment for those with acne scarring. It uses a chemical substance to stimulate skin regeneration. However, chemical peels come with their own set of risks and can even exacerbate scars if not performed correctly. If you’re considering a chemical peel to help with your acne scarring, it’s important to seek the advice of a board-certified dermatologist and make sure a licensed medical aesthetician administers it.

“Chemical peels are a great option for someone with scarring in its early stages. It can help up to six to twelve months after a scar is formed, but it isn’t as effective for older scarring. I recommend glycolic peels or the afa® Clay Peel, which incorporates bentonite clay and a proprietary ingredient called afaLUXE. This ingredient works similarly to glycolic acid, but patients tend to experience less irritation and side effects. It is gentle and can help with active acne, some scarring and post-inflammatory hyperpigmentation,” Dr. Joel Schlessinger says.

Acne scar laser treatment

Lasers can be effective for certain kinds of acne scarring. Depending on the type and extent of acne scarring, as well as the patient’s skin type, acne scar laser treatment can be effective. Single or multiple laser types may be used depending on your dermatologist’s guidance.

Exercise the same caution when getting a laser treatment as you would with microneedling or chemical peels. However, laser treatments can potentially exacerbate symptoms if not performed correctly.

“For redness, IPL or BBL can be quite helpful. For textural concerns in skin of color, MOXI® is safe and effective, though it is gentler and will require a series of treatments over time to achieve maximal results. For lighter skin types, HALO, Erbium ProFractional, Erbium TRL and CO2 may all be reasonable treatment options,” Dr. Daniel Schlessinger says.

The type of laser you and your dermatologist decide on depends on your skin tone, your overall concerns and the type of scarring. Each laser works on a different wavelength and with different strengths, which is why it’s important to discuss it with a board-certified dermatologist.

Lasers, microneedling and chemical peels aren’t the only solutions for acne scars.

“Depending on the type and extent of acne scars, subcision or fillers may be great options as well. Subcision involves numbing the area of scarring with lidocaine and using a needle to enter the skin from beside the scar to release the tethering that pulls skin down. It can be quite effective. Sometimes, it is done in conjunction with fillers or laser resurfacing for even better results,” Dr. Daniel Schlessinger says.

“As far as deeper scarring goes, it remains a challenge even with invasive treatments like lasers and peels,” Dr. Joel Schlessinger adds. “Topical sheets of silicone or silicone-based gels such as Stratpharma products can help in the early phases of healing,” he says.

The best products for acne scars

Some acne scars aren’t actual scars but simply pigment left behind by a breakout. “We call that post-inflammatory hyperpigmentation, or PIH for short,” Dr. Joel Schlessinger says.

“PIH can be improved by products that target pigment and improve cell turnover. We recommend LovelySkin LUXE Clarifying Gel Cleanser, along with the LovelySkin LUXE treatment gels, which are made in graduated strengths. The cleanser and treatment gels are made with the same ingredient afaLUXE found in the afa® Clay Peel, but are designed to be used daily.

We recommend starting with Acne Care Gel 9% or Exfoliating Gel Mild 11%.
Sometimes, a good skin care routine isn’t enough to clear up acne. You may need antibiotics or other acne treatments that can only be prescribed by a physician or dermatologist. One of those treatments is known as Accutane, or isotretinoin.

Since acne is a unique condition, and symptoms vary from person to person, you and a board-certified dermatologist should discuss whether Accutane is right for you.

Drs. Joel and Daniel Schlessinger, board-certified dermatologists, Mohs surgeons and cosmetic surgeons, have years of experience evaluating and treating acne for the best patient outcomes. In this blog, we’ll discuss details of Accutane treatment, including:

How does Accutane work?

Accutane is a very concentrated form of vitamin A that comes in capsule form. Vitamin A is found naturally in the body and in some foods, but taking this form of it by mouth in a highly concentrated dose is incredibly effective at treating acne. “Accutane works by reducing the sebum that is in your glands, which in turn decreases the ‘food’ for acne and the normal inhabitants of your acne-related glands,” Dr. Joel Schlessinger says.

Some forms of acne can be treated with prescription-strength skin care products and antibiotics, but if your acne isn’t responding to treatments like these, it may be time to consider Accutane. While it can be greatly beneficial, it may not be the best fit for everyone.

“We don’t prescribe Accutane to individuals who are unwilling or unable to guard against pregnancy and are people of childbearing age, as isotretinoin can cause severe birth defects. We are also cautious of prescribing it to individuals with serious concerns for depression or suicide, only using it in coordination with their psychiatrist or mental health professional,” Dr. Joel Schlessinger says.

It’s ideal for moderate to severe acne that has not improved with other treatments. Depending on the case and severity, it can also be beneficial for acne scarring.

How long does Accutane take to work?

As part of your daily routine, patients generally take it once to twice daily. The dosing is dependent on your body weight, and how long you take it depends on your unique concerns and results.

“Usually, we will start to see benefits within the first month or two, but to get the best results, you should stay on it for at least five to six months at a fairly high dose. This will often result in remission (complete improvement) of acne for at least six months to a few years. Often, it will lead to a complete cure if hormones are in play and patients are nearing the end of massive swings in hormones of adolescence,” Dr. Joel Schlessinger says.

Dr. Daniel Schlessinger adds, “Since Vitamin A is a fat-soluble vitamin, it’s important to take it with a fatty meal, so it can be fully absorbed into your body. Although, some versions are better absorbed even without a fatty meal now. Our clinic worked on one, Absorbica, during their clinical trials.” It is also recommended to take it with a full glass of water to avoid irritating the esophagus.

What is an Accutane purge?

When you first start taking isotretinoin, you may experience what’s called an “Accutane purge.” An Accutane purge is a short period that occurs as you start taking isotretinoin, when breakouts may increase.

“We have a special approach to starting Accutane that often totally avoids this purge, leaving people with a smooth transition from acne to no acne. It is rare for other practices to do this, but we find more than 90% of our patients never have a purge,” Dr. Joel Schlessinger says.

“This approach involves treating any bacterial or yeast infections prior to Accutane and a gentler approach to the initial Accutane dosage, among other methods we use,” he continues.

Pros and cons of Accutane

Accutane can be a major help when it comes to persistent acne. “Bar none, it’s the best treatment for acne,” Dr. Daniel Schlessinger says. “But it also has many side effects to monitor for, some of which are serious, such as potential for worsening depression or suicidal thoughts, an increase in inflammatory bowel disease symptoms and the risk for brain swelling that leads to headaches and blurry vision. For this reason, it isn’t the first thing we reach for when we see someone with acne and we usually try to calm down the acne as much as we can with other methods before starting Accutane. In the right patients, though, it is an incredible treatment that avoids or even reverses scarring and serious acne-related skin issues.”

“When administered correctly,” Dr. Joel Schlessinger continues, “Our patients usually see great improvements. There is an art to it, so not every practitioner administers it the same way and achieves the same results.”

Invariably, patients will have dry skin and some joint pains, which can be easily managed and don’t cause too many problems. However, it does require some guidance. Dr. Daniel Schlessinger came up with the idea for FixMySkin 1% Hydrocortisone Healing Balms when he took isotretinoin as a teenager to combat some of those concerns.

“Dealing with the dry, cold winters compounded with the effects of isotretinoin, I was in need of an easy solution,” Dr. Daniel Schlessinger says. “We put all of our Accutane patients on FixMySkin, and they love it. FixMySkin can be used on the face, hands, lips and body, so any areas of irritation can be soothed,” he continues.

Joint pain is also common but can usually be improved with medications like ibuprofen or other non-steroidal anti-inflammatory agents.

One of the most important things to consider with Accutane, though, is the necessity to avoid pregnancy during treatment. “Accutane is teratogenic, which means it can cause severe birth defects to an exposed unborn fetus,” Dr. Joel Schlessinger says.

All patients taking Accutane are required to enroll in the the iPLEDGE Program, which is a federally mandated system to ensure safety around Accutane usage. For patients who are capable of becoming pregnant, the program requires monthly pregnancy tests throughout treatment and the pledge to use two forms of birth control during and before treatment.

“It’s also integral to keep an eye on your mood during Accutane treatment, as depression and even suicidal ideation can be a concern. With appropriate concern and management, most problems with Accutane are easily avoided or treated,” Dr. Joel Schlessinger says.

Lastly, Accutane can interfere with wound healing and increase the risk of scarring for procedures performed during Accutane usage. “We recommend that our Accutane patients avoid elective procedures or trauma to the skin, including wisdom teeth extractions, tattoos, piercings or laser resurfacing during the course of treatment and for several months after finishing the treatment,” says Dr. Daniel Schlessinger.

Accutane is usually an affordable option when purchased with health insurance. Even without insurance, it costs about $150 a month at discount pharmacies.

What’s the difference between tretinoin vs. isotretinoin?

There are many forms of vitamin A, some of which can be taken orally (like Accutane). There are other forms of vitamin A you can apply topically, like tretinoin or differin, which are retinoids. In fact, many dermatologists recommend starting a retinoid after Accutane treatment to extend your results. If you’re wondering about the difference between tretinoin vs. isotretinoin, think of them as an oral treatment vs a topical treatment. Tretinoin, or Retin-A, is a topical form you apply to your skin at night. Isotretinoin, however, is the capsule form you take twice daily known as Accutane.

Accutane before and after

While individual results will vary based on the severity of your acne or scarring, we’re familiar with good Accutane results at Schlessinger MD. We have been treating acne patients for over 25 years and have many Accutane before and after photos to show the range of results.

When a breakout or blemish appears on your skin, it can be difficult to determine the cause. While some cases of acne are hormonal, there are other factors, including bacterial infections, that can also contribute to acne. To complicate matters more, they are not mutually exclusive and your acne can be both hormonal and bacterial.

Board-certified dermatologist, cosmetic surgeon and Mohs surgeon, Dr. Daniel Schlessinger explains, “While mild acne or scattered comedones and blackheads are often the result of a hormonal imbalance or clogged pores, many, many cases of acne are actually caused by bacteria or yeast living on the skin.”

Navigating such a common, yet complex skin condition like acne can be challenging, but scheduling a visit with a board-certified dermatologist is your first step to understanding the source of your breakouts and how best to treat them.

In this article, we’ll explore the key differences between various types of acne, how common sources of bacteria can contribute and help you understand how to identify, manage and treat these skin conditions properly.

Common causes of acne and skin infections

There are many causes of acne, including hormonal changes, skin hygiene, hereditary factors and skin infections. Acne forms when pores in the skin are exposed to bacteria, like Propionibacterium acnes, and/or become clogged with excess oil and dead skin cells. Since acne-causing bacteria can be found on everyday surfaces, there are many reasons for acne to occur on the face or body. Determining if acne is hormonal, bacterial, fungal, or a combination depends on the symptoms.

Dr. Daniel Schlessinger, a board-certified dermatologist, as well as Mohs surgeon and cosmetic surgeon, explains, “Hormonal acne is caused by shifts in hormone levels, often triggered by puberty, menstruation, pregnancy or certain medications. Bacterial acne often occurs after contact sports like wrestling, working out in an unclean gym or even after a visit to a hospital or nursing home. Yeast (or fungal) acne is also very common and can occur in individuals who sweat more frequently. However, there are times when the source of the infection remains unknown.”

Dr. Joel Schlessinger highlights the prevalence of bacterial skin infections, saying, “These infections are very common and can happen to anyone, especially if you’ve been in contact with germs or your skin barrier has been injured or broken. Practicing good hygiene like frequent (but not too frequent) handwashing, disinfecting and covering wounds and being mindful of what you touch throughout the day can help reduce the risk of infection.”

Understanding your acne

The symptoms of acne can vary from mild to more severe, depending on the type. Most people are familiar with pimples, which are small, red, inflamed bumps on the skin. You may also experience blackheads, which occur when pores are clogged with oil and dead skin cells but remain open, giving them a dark appearance. In contrast, whiteheads form when these clogged pores are sealed, creating small white bumps.

Nodules or cystic acne are deep, often painful, bumps that can lead to scarring if not properly treated. It’s important to remember that acne can appear on many different parts of the body, not just the face. Body acne may occur on the back, chest or shoulders.

Hormonal acne often presents as painful, red cysts and often appears on the lower half of the face, including the jawline and chin. Bacterial acne tends to appear on the surface of the skin in the form of pimples or pustules and pops up in oilier regions of the face like the forehead or cheeks. Yeast acne usually looks like numerous tiny, whitish bumps on the face, chest and/or back.

More seriously, a skin infection might exhibit similar symptoms to acne initially, but an infection may feel warm, itchy or painful. Skin infections can occur alongside acne, especially if bacteria enter the skin through an open blemish, but they can also appear on healthy skin. If the infection spreads, you may notice additional symptoms like fever or chills. Acne that appears suddenly in people who are well past the typical age of acne likely have an infection and obtaining a bacterial culture may be something your dermatologist does when you visit their office.

Dr. Daniel Schlessinger explains, “Under rare circumstances, bacterial infections can impact deeper layers of skin and even enter the bloodstream if left untreated. If you notice any of these signs, especially fever, a streak emanating from a cyst or more widespread redness, it’s important to seek medical advice to prevent complications.”

Everyday sources of acne-causing bacteria

It’s easy to overlook some of the daily tasks that may contribute to breakouts. In fact, many common activities and environments can expose your skin to harmful bacteria or irritants that exacerbate breakouts.

Hitting the gym is great for your health, but it can also pose unexpected risks to your skin if you’re not careful and the gym isn’t cleaning equipment regularly. Avoid touching gym equipment and then touching your face or body, especially while sweating. This can transfer bacteria to the skin, contributing to acne.

Dr. Daniel Schlessinger says, “Using a body wash with sodium hypochlorite, like the CLn Sportwash, can be very beneficial for reducing bacterial infections after playing sports or working out at the gym, as well as for anyone experiencing “bacne” or folliculitis.”

Other common culprits include dirty makeup brushes, which are a breeding ground for bacteria if not cleaned often, unwashed pillowcases, and environmental factors, including pollution and humidity. Dietary choices, such as cow’s milk, which may contain hormones, can also contribute to acne.

Also, protein shakes and muscle building supplements can cause acne due to the hormones these often contain. Testosterone, in both the injectable and topical forms, can cause significant acne, particularly on the back. Oftentimes, an adjustment of dosage on testosterone can greatly improve acne. Finally, picking at your blemishes not only makes acne worse but also introduces bacteria into the open skin, potentially leading to an infection or scarring.

Dr. Joel Schlessinger suggests, “Simple changes like regularly cleaning your makeup tools, avoiding touching your face during workouts and sports activities, and ensuring that gym equipment is cleaned regularly before use can greatly reduce the risk of exposure to acne-causing bacteria.”

Finding the right treatment

Effective treatments are available to help treat acne and eliminate bacteria on the skin. For mild acne, topical treatments formulated with salicylic acid or glycolic acid can provide exfoliation to help unclog pores. LovelySkin LUXE Acne Care Gel 9%, which contains a blend of amino-based filaggrin antioxidants, called afaLUXE, improves skin’s ability to retain moisture while gently exfoliating skin. Great for oily, acne-prone skin, it eliminates existing blemishes while working to prevent future breakouts from forming.

Looking for the best face wash for hormonal acne? We recommend LovelySkin LUXE Clarifying Gel Cleanser, which gently exfoliates the skin with acne-fighting salicylic acid, glycolic acid and afaLUXE. Vitamin C and Dead Sea minerals nourish and rebalance the skin.

Dr. Daniel Schlessinger explains, “There are many effective over-the-counter acne treatments that can help treat and prevent breakouts. It’s important to keep in mind that patience and consistency are key to achieving long-term results. However, if acne persists or becomes painful, it’s best to see a dermatologist for professional acne treatment as soon as possible.”

If bacteria is determined to be contributing to your acne, the treatment path is a little different. Dr Joel Schlessinger explains, “In some cases, antibiotics are necessary to treat the underlying source of bacteria that is contributing to your acne. Antibiotics can be very effective, but it’s important to know what is being treated and treat it for the full course.”

When you’re dealing with acne, it’s important to remember these conditions are very common and manageable, and you don’t have to face them alone. At Schlessinger MD, our providers will carefully examine your skin to determine the cause and recommend the best course of treatment for your specific needs.

Our board-certified dermatologists may suggest over-the-counter treatments, prescribe antibiotic pills or recommend prescription-strength options to address more severe skin infections or painful cystic acne.

Joel and Daniel Schlessinger or one of our physician assistants by visiting our appointment page and booking online.

While humidity can help relieve dry skin by boosting hydration, it can also wreak havoc on oily and acne-prone skin by contributing to breakouts. Board-certified dermatologist, Mohs surgeon and cosmetic surgeon, Dr. Joel Schlessinger explains, “During summer months, the temperatures rise and so does the humidity. This triggers sebaceous glands to produce more oil which mixes with sweat, dirt and dead skin cells, which congests and blocks pores. This leads to a shiny complexion and more breakouts, blackheads and whiteheads.”

Managing acne any time of year can be no simple task, but it can be especially difficult when the heat and humidity are working against you. Fortunately, taking a few specific measures can help. In this article, we’ll discuss:

How to prevent acne during the summer

Bacne treatment tips for the summer months

Summer lifestyle changes that can help acne

When to see a dermatologist

How to prevent summer acne breakouts

Proper hygiene is one of the best ways to manage acne. If you aren’t committed to a skin care regimen designed for oily or acne-prone skin, summer is a great time to start. During hotter and more humid months, we sweat more, expose our skin to chlorine, salt water and sand and wear sunscreen more often, which is very important for skin health, but may clog pores if it’s an oil-based formula.

Board-certified Physician Assistant, Jackie Gaffney, MPAS-C recommends taking these preventative measures, “Please remember to wash your face twice daily, avoid touching your face, use a moisturizer and always wear sunblock.”

Cleansing twice a day with a 2% salicylic acid face wash, like the LovelySkin LUXE Clarifying Gel Cleanser, can help remove excess sweat, oil and dirt from pores while providing gentle exfoliation with 3% afaLUXE and 2% glycolic acid.

Another important part of keeping acne at bay during summer months is adding a daily exfoliator to your regimen. Board-certified Physician Assistant, Shea Perillo, MPAS-C explains, “With summer comes increased humidity, so it is important to add in an exfoliating treatment once a week to help slough off dead skin cells that may contribute to acne. Choose a gentle exfoliating product to unclog your pores and save your skin from unwanted breakouts.”

An exfoliating treatment, like the LovelySkin LUXE Exfoliating Gel Mild 11%, is a great choice for summer months because it improves acne with 11% afaLUXE, providing deep yet gentle exfoliation to clear clogged pores and help skin slough away dead skin cells.

Additionally, while it may seem like skipping moisturizer to let skin “dry out” is a good idea when skin feels oily, it’s not. Not properly moisturizing any time of year can cause skin to overcompensate and create even more of its own oil. Choose a light moisturizer, like the LovelySkin LUXE Mattifying Antioxidant Moisturizer, to keep skin hydrated, protected and soft, but steer clear of heavy moisturizers during humid summer months.

Dr. Daniel Schlessinger, a board-certified dermatologist as well as Mohs surgeon and cosmetic surgeon, says, “Be careful with thick, occlusive moisturizers and makeup or when wearing face masks, as these all trap moisture in the skin and may cause an acne breakout.”

During the daytime, always follow your moisturizer with an oil-free sunscreen made for acne, like EltaMD UV Clear Broad Spectrum SPF 46 Facial Sunscreen. This sunscreen is available in untinted, tinted and deep-tinted formulas, won’t contribute to breakouts and also exfoliates and calms blemish-prone skin with lactic acid and 5% niacinamide.

Bacne treatment tips for the summer months

Acne on your back, or “bacne,” can become more prevalent during the summer months. Increased sweating from higher temperatures and friction from sweaty clothing, backpacks or sports equipment can all contribute to acne on your shoulders and back. Dr. Joel Schlessinger adds, “Humid weather can be hard on acne, and back acne often flares when this happens. Washing with CLn SportWash can help greatly when sweat leads to acne.” This body wash can be used daily or intermittently as needed after exercise or sweating and is formulated with sodium hypochlorite, also known as bleach, to clear skin of bacteria that contribute to acne. Choosing moisture-wicking fabrics such as lightweight cotton instead of polyester or nylon, which traps and holds sweat and moisture onto skin, can also help prevent bacne.

Summer lifestyle changes that can help acne

In addition to your skin care regimen, a few lifestyle changes can help you effectively manage acne during humid months. Here are our top tips:

Limit time outside. Not only does this help minimize the risk of sun damage and sunburn, but it also minimizes sweating which contributes to acne on the face and acne on the back.

Remove wet swimsuits right away and rinse after swimming. Wet, tight-fitting bathing suits can cause friction and lead to rashes or bacne.

Wash your sheets and pillowcases frequently. Dirty linens can transfer bacteria to your skin and contribute to acne.

Hydrate from the inside out. Drinking enough water daily has a myriad of benefits, but for acne, properly hydrating helps skin rid itself of toxins that contribute to acne.

When to see a dermatologist

In some cases, over-the-counter products just aren’t enough to manage acne, especially during the summer. If your at-home efforts aren’t working, it may be time to see a dermatologist. Our providers will examine your skin and help determine both the probable cause of your acne and the best course of treatment for you. Additionally, fungal acne, which is more prevalent during the summer months due to increased sweating and humidity, will need to be diagnosed by a dermatologist and may need prescription treatments.

Our dermatologists may also prescribe antibiotic pills or prescription-strength retinoids, like Accutane, especially if your acne is severe, cystic or nodular. Additionally, we may recommend laser treatments, like BBL Forever Clear, which uses a combination of blue, yellow and infrared light to eliminate bacteria in pores to help clear acne and minimize redness.

If you’re dealing with increased acne this summer, you can reserve an appointment with Dr. Joel Schlessinger or one of our physician assistants at Schlessinger MD by visiting our appointment page and booking conveniently online.

Acne is one of the most common skin conditions a teenager can face, but along with it often comes feelings of insecurity about their skin’s appearance. While nearly 85% of teenagers have acne as a normal part of growing up, that doesn’t mean it’s an easy experience

Dr. Joel Schlessinger, a board-certified dermatologist, Mohs surgeon and cosmetic surgeon and our team of physician assistants recognize the complexity of teenage acne. Dr. Schlessinger says, “I’m very results-oriented and take great joy out of being able to help teens and other patients who suffer from acne or sometimes what appears to be acne but is actually an infection masquerading as acne. There are so many times that I see patients who have been everywhere for their acne and we are their last resort; it gives me pleasure to see them back with a clear complexion and no evidence of the acne they had just weeks before. To me, that is the reason I went into dermatology.”

He continues, “Often, we will be the first dermatologist they have seen who really looks at their face and tries to figure out the reason for the acne rather than putting them on the same pills for months on end without any signs of improvement. My staff and I are going to work our best to get to a better place within several months or even sooner.”

What is teenage acne?

Acne is a complex skin condition with various underlying causes, often stemming from multiple factors. Dr. Daniel Schlessinger, board-certified dermatologist as well as Mohs surgeon and cosmetic surgeon elaborates, “Dietary elements such as dairy products and hormone-rich foods, alongside genetic predisposition, ineffective cleansing routines, bacterial infections and a form of ‘yeast acne’ can all contribute to acne. Interestingly, many dermatologists are also seeing more acne due to the increasing pollution in our environment. Sometimes, it’s the culmination of several factors, or a skin condition can appear as acne, but be a different issue altogether.”

Studies on teenage acne and adult acne in women show it often stems from hormonal imbalances during onset of puberty or major life changes like pregnancy or menopause. This period triggers significant fluctuations in estrogen, progesterone and testosterone levels, which can lead to increased oil production in the skin. When this excess oil combines with dead skin cells, it can block pores and result in acne. While not universal, hormonal pimples commonly appear on the lower face, including the chin and jawline, but can occur all over the face as well as the back. Additionally, recurrent breakouts in specific areas, particularly around the time of menstruation, indicate a hormonal component to the acne

For teenagers, types of pimples can present in a couple ways or varying combinations of all three:

· Comedonal acne: This type of acne can appear when pores get blocked with a mixture of sweat, oil, dirt and other debris and form blackheads and whiteheads.

· Inflammatory acne: This type of pimple presents as red, swollen, painful pustules, nodules or cysts that contain bacteria, dead skin cells and excess oil.

· Cystic acne: This is a severe type of inflammatory acne that often presents as pus-filled cysts deep under the skin. These types of pimples are often painful and can be as small as a pea or large as a dime. This form of acne is the worst kind and can often lead to scarring, so it is important to get ahead of it as soon as possible so damage isn’t permanent.

Our approach to teenage acne at Schlessinger MD

When is it time to call in the experts? While there are many over-the-counter treatments you can try at home, navigating the complexity of this skin condition often warrants expert guidance from a dermatologist. Starting your acne treatment under the supervision of a dermatologist is typically the most effective approach. Without expert advice, you might invest time and money in products that fail to target the underlying causes of your acne, potentially leading to permanent scarring from untreated lesions. There is a limited time between when acne becomes a problem and when long-term or forever scarring occurs, so it is important to get in early to see a dermatologist.

At Schlessinger MD, we adopt a comprehensive approach to addressing various forms of acne, ranging from mild to severe cases, including infections and any accumulated acne scarring. Our medical professionals tailor treatment plans to suit your specific needs after assessing your lifestyle factors, past treatments and medical history. Our team will discuss treatment options with you, which may involve a combination of topical and oral antibiotic, retinoid and anti-androgen prescriptions. At times, Dr. Schlessinger may refer you to our Certified Medical Aestheticians in the med spa to incorporate professional treatments like laser treatments, in addition to the recommended personalized at-home skin care.

Our signature at-home acne regimen features a unique dermatologist-developed molecule known as afaLUXE. Dr. Joel Schlessinger explains, “We developed this innovative ingredient complex to provide effective exfoliation and eliminate pore-clogging debris while maintaining optimal moisture levels. This is particularly beneficial as it prevents the common issue of dryness associated with more traditional acne medications.” Here is a closer look at our LovelySkin LUXE acne regimen:

LovelySkin LUXE Clarifying Gel Cleanser: What makes this cleanser great for acne is in addition to the 2% glycolic acid and 2% salicylic acid, it’s also formulated with 3% afaLUXE. This unique antioxidant molecule improves cellular turnover and exfoliation but also works in synergy with the skin’s moisture barrier. This ensures virtually no irritation, making it an ideal cleanser for oily and acne-prone skin. You can also use this cleanser to purify acne-infected skin on the back.

LovelySkin LUXE Dual Action Toner Pads: These toner pads are a great follow-up to the cleanser as they combine 5% glycolic acid and 5% salicylic acid in a pre-soaked, easy-to-use pad that swipes easily over any part of the body.

LovelySkin LUXE Acne Care Gel 9%: This gel is gentle yet effective at not only eliminating different types of pimples but also helping to prevent them from occurring in the first place. 9% afaLUXE provides effective exfoliation while helping to balance and support skin’s protective barrier. Unlike a spot treatment, this gel is designed to be applied all over the face to treat and prevent acne.

LovelySkin LUXE Mattifying Antioxidant Moisturizer: Contrary to popular belief, acne-prone and oily skin types do need a moisturizer. If skin is left to dry out, it will overcompensate and produce even more oil, which will only intensify your acne. We recommend this product for our acne patients because it helps exfoliate with afaLUXE, protects against pollution with antioxidants and leaves skin with a soft, matte finish. It’s also suitable for use in conjunction with Accutane, a common prescription for severe acne.
